For those of you who were unable to attend the Prospective CHS Volleyball Players & Parents zoom call, please see the slides from our call with the following highlights:
- Tentative tryout date – Saturday May 1, 2021
- Register for tryouts – https://app.picklejuiceapp.

- Physicals – The girls have to have their physicals BEFORE tryouts
- Commitment night – Week after tryouts
Regarding the physicals…. there are 3 forms in Dragon Fly Max that need to be submitted- 2 need to be signed by a healthcare provider (PE & Medical Eligibility form). The 3rd Health History form is something you fill out and submit electronically via DragonFly Max, HOWEVER a lot of pediatricians (mine included) like to see it when they’re completing the PE.
All 3 forms are in DragonFly Max, but I’ve attached them here for your convenience. Please complete them and upload them in your DFMax account prior to tryouts.
